You can obtain form HS22X from any Health Centre or from the Central Services
Agency (CSA). You should fill it in and then take it to register with a doctor
(see below).
If you need any help to fill in this form or you need an interpreter you can contact any of the ethnic minority community groups listed below or the Multi-Cultural Resource Centre for further assistance.
The CSA can advise you which G.P.s are closest to the area in which you live.
It is very important to register with a G.P. because they are often the person
who arranges first contact with health and social care services. If the G.P. accepts
you, he or she will sign your HS2XX form and you will receive your medical card.

REGISTER WITH A DENTIST
To register with a dentist (G.D.P) you must have a Northern Ireland Medical Card.
To choose your dentist you can obtain lists of dentists from the:
PHARMACY SERVICES – THE CHEMIST
Your Doctor (G.P.) is the person who decides which medicine you need. He or she will give you a form (called a prescription) giving you permission to buy the medicine in a pharmacy. Without it your chemist cannot give you medicines, except for some very simple medicines (such as painkillers) which can also be obtained in supermarkets and other stores. Medicines dispensed with a prescription all cost the same (currently around £6.50). However you do not have to pay if:

you must fill in form HC1 in your local Social Security Office.
